How to attest to IA for Group Practices

  1. Login to Patient360
  2. Navigate to Group Practices 
  3. Select your Practice
  4. Click on the Template/Manual Entry button
  5. Navigate to the IA measures tab
  6. Click on Add Measure to search and add your measure(s) 
  7. Once your measure(s) have been added, click on the measure name then the Report Measure button
  8. Select the check box → click Report Measure
  9. Confirm there is a checkmark under complete





How to attest to IA for Individual providers

  1. Login to Patient360
  2. Navigate to Providers → Providers
  3. Select your Provider's name
  4. Click on the Template/Manual Entry button
  5. Navigate to the IA measures tab
  6. Click on Add Measure to search and add your measure(s) 
  7. Once your measure(s) have been added, click on the measure name then the Report Measure button
  8. Select the check box → click Report Measure
  9. Confirm there is a checkmark under complete
