SFTP Export Reports Overview
There are six different SFTP Reports that can be set-up to automatically deliver to Patient360's SFTP weekly, bi-monthly or monthly. These reports are: QPP Program Participation Report, Measure Performance Summary by Group, Measure Performance Summary by Provider, Measure Performance Patient Detail, Care Gap Report, and Subgroup Report.
Please see below for instructions how to request one or more of these reports and for details on what each report will contain.
How to Request SFTP Export Reports
To request one or more of the SFTP Reports listed below, please reach out to support@patient360.com and include the following information:- List the SFTP Reports you would like to receive
- Include the frequency you'd like to receive these reports:
- Weekly (on Monday's)
- Bi-monthly (on the 1st and the 15th of the month)
- Monthly (on the 1st of the month)
- Specify how the Performance Reports should be delivered to SFTP:
- Individual csv's in a subfolder
- Individual csv's in a .zip file

QPP Program Participation Report
- This report lists participants with estimated category and program scores

| Exclusion | Count of instances meeting exclusion criteria for the group/measure |
| Exception | Count of instances meeting exception criteria for the group/measure |
| Not Reported | Count of denominator-eligible instances for the group/measure with no numerator response |
| Reporting Rate | Reporting rate for the group/measure |
| Benchmark Score | Benchmark score based on performance rate |

- This report lists patient-level detail for which the quality action was not completed successfully
