Veradigm M112 2025 Decision

Veradigm provided a final decision that they were not going to support M112 for the 2025 reporting year. They did provide a link (https://help.practicefusion.com/s/article/how-do-i-create-an-ehi-export) Patient360 is exploring the use of this file to import eCQMs and/or CQMs.

I am circling back to update you on our internal review of Practice Fusions support for CMS125v13.  Currently, Practice Fusion is unable to reinitiate CMS125 into the 2025 reporting dashboard for inclusion in the QRDA file. 
 
As discussed, PF removed support of CMS125 as of January 2024 when CMS removed it from the Traditional MIPS program.  We notify our customers of the eCQMs that Practice Fusion supports each year.  Those notifications with links to the documentation were delivered on May 23rd, 2024, and February 27th, 2025.  I am including those links below. 
 
 
 
I will continue to explore alternative options internally, however the EHI export functionality, which is currently available, may be helpful in support of your MIPS CQM reporting.  https://help.practicefusion.com/s/article/how-do-i-create-an-ehi-export

Denominator Validation

TIN-level validation with Patient360 can be accomplished by running the Patient List report in Practice Fusion on the Clinical Reporting screen. This report provides multiple criteria options to allow searching for patients based on encounter date, diagnosis, and date of birth.


eCQM Template Conversion

If QRDAIs aren't available for any supported measures, Patient360 can convert two specific Practice Fusion reports into eCQMs (pf_rpt_to_ecqm.py). 

  1. Create a group for all providers or generate a file for each file using Reports->Patient List, Start query, age, (use 1 year lower and 1 year higher than what’s required) or all ages, Export to CSV. Be sure to choose All Patients with Encounters in the performance period. This needs to be done for each measure. Include the Measure ID (001, 112, etc) in the file name.
  2. Reporting->eCQM Dashboard->Click Measure->Download CSV
