This resource is meant to provide ACO Member TINs an opportunity to assert eCQM performance for their patient population when they are unable to generate QRDA-I files and are using a 2015 CEHRT EHR.
To accomplish this, Patient360 has developed a template to collect eCQM data sufficient to sync and de-duplicate when aggregating at the ACO level.
ACO Member TINs who opt to submit eCQM data via template must create individual .csv files for each measure (see attachment below). When filling out your templates, please follow the guidance below:
